A psychopath is called out because of their harmful actions which is a warning to others and a (possibly futile) attempt to make them recognise the harm they're doing and stop their abuse. 

Calling someone fat, though it may be a matter of fact, is useless. Whether the cause is poor diet, genetics, sedentary lifestyle - it's just the way they are and unless you're their doctor and being asked for advice, there's nothing worth saying about it. Putting on weight does not equate to moral decline.

The chubby kid who is useless at sports - is that really any reason to denigrate them? Sure, kids can be cruel, and I expect young Prem got his share of that, but some of us grow out of it. Sticking the label on yourself and calling yourself fat and ugly doesn't give you a carte blanche to sticking it on anyone else. That in itself is ugly.
