It's possible to be involved with either drugs sex, or rock'n'roll or all three and still have empathy. Prem's reaction to punish ( by shutting off contact, what could be worse after all) the plea to help the starving babies or a plan to actively help the destitute and mentally unstable is obviously the part where the veil fell off for this guy.
We have all wished it fell off way before it did and I believe 40 years of journaling and other processing he mentioned should be long enough to not be haunted by remorse.
